Individuals can order books by phone (see numbers below) using a credit card for payment, or by mail or fax using this form. To use the form, print it, fill it out, and then either:
• Prepare a check or money order for the total amount in U.S. funds payable to “Slavica” and mail it with the form to Slavica at the address below; or
• Fill in complete credit card information and either mail or fax the form to Slavica.
Allow about two weeks for delivery within the U.S., longer if overseas.  

Foreign customers: Slavica recommends using a check drawn on a U.S. bank, indicated by routing numbers along the bottom. If you send a check that is merely referred to a U.S. bank (no routing numbers), and the bank charges a collection fee, your payment must include the collection fee.
